The purpose of this survey is to assess Minneapolis food workers and managers:

• Food allergy knowledge
• The impact of food allergy information and resources provided by the City

Your responses will help us to understand:

• Knowledge, attitude and practices around food allergens
• Need for more food allergen awareness training or other resources
• Effectiveness of City services, information, and resources about food allergens

This short survey is voluntary and should take approximately five minutes to complete.
