Benefit maximum amounts

If you use the services of a Delta PPO dentist, the plan will pay up to $1,500 per calendar year for each covered person. If you use the services of a Delta Premier or out-of-network dentist, the plan will pay up to $1,000 per calendar year for each covered person.

Orthodontics is subject to a separate lifetime maximum of $1,000 per covered person and limited to those orthodontic treatment plans beginning on or after the eligible dependent child’s eighth birthday and through the eligible dependent child's eighteenth birthday.

Deductible

An annual deductible of $50 per year for each covered person applies to basic, major, and prosthetic services. The annual deductible will not exceed $150 per family. The annual deductible does not apply to diagnostic, preventive, or orthodontic services. The plan document has detailed information on the eligible covered services and how to use these benefits.

Enrolling for dental benefits

Most full-time employees and their eligible dependents may enroll in benefits the first of the month following the first day of employment. Certain part-time employees are eligible to participate in the dental plan. Eligibility and contribution requirements for part-time employees are based on the governing policy or collective bargaining agreement.
