All City Council Operating Departments. Independent Boards and Commissions may elect to participate with City’s Workers’ Compensation self insurance program or purchase Workers’ Compensation Insurance commercially. MN Statute 176 applies to all employers providing Workers’ Compensation insurance within the state of Minnesota. |
Synopsis |
Employees reporting a Workers’ Compensation (WC) claim must complete a Supervisor’s Report of Injury (SRI) form and present it to their Supervisor immediately. Supervisor will review SRI for accuracy and forward it to Payroll. Payroll will add pertinent salary information and transfer data onto a First Report of Injury (FRI) form. Payroll then forwards the SRI and FRI to Finance/Risk Management Office for action. WC Claim Coordinators review the claim and make a determination whether to accept or deny based upon available information. In addition, WC Claims Coordinators notify the State (as mandated by statute) of those claims incurring lost time. |
