Sidewalk inspection permit form

Before you begin sidewalk repairs, you must fill out the sidewalk inspection permit form.

What to know

You must complete this form if you want to build or fix any of the following:

  • Sidewalks
  • Curbs or gutters
  • Driveway aprons
  • Pedestrian curb ramps

Forms you may need

You will need to attach copies of these documents:

  1. Your current Certificate of Liability Insurance document*
  2. Your Cold Weather Concrete Protection Plan
  3. Your Site Plan

*Certificate of Insurance on file

  • If you're doing work in the City for the first time, you must attach a certificate of insurance when you apply.
  • Contractors must keep a current certificate of insurance on file with the City.
  • After renewing insurance, all contractors must attach their updated insurance information.
