- Find the form for the permit you wish to apply for.
- It will be a fillable PDF.
- You will need to have Adobe Acrobat or Adobe Reader to open it.
- Save the blank application on your computer or device.
- Complete the application and save it again to your computer or device.
- Be sure to include your first and last name on the application.
- Attach a copy of the completed application and email it to us.
- Do not include any other attachments to the email.
